The Madison Local Schools are accepting applications for open-enrollment during the month of May for the 2018/2019 school year.
The child’s parent/guardian will need to come to the Madison Board of Education, 1379 Grace Street, Mansfield, between the hours of 8:00 AM – 4:00 PM.
You will need the following items to apply:- Childs birth certificate
- Immunization record
- Proof of residency (utility bill)
- Custody papers if applicable
Parents will be notified by letter if their child is accepted into the open-enrollment program. Please call 419-589-2600 with any questions.
