The Ohio District 5 Area Agency on Aging, Inc. is offering a series of vaccine clinics throughout October to ensure equitable access to COVID vaccines for local residents. These clinics are open to anyone age 18 and older, regardless of county, and will be held at four locations across the region.
Clinic Locations and Dates:
- October 4: Hawkins Conference Center, 2131 Park Avenue West, Ontario
- October 11: Third Street Family Health Services, 600 W. Third Street, Mansfield
- October 18: Ashland Health & Wellness Center, 309 Arthur Street, Ashland
- October 25: Bucyrus Health & Wellness Center, 2458 Stetzer Road, Bucyrus
All clinics run from 9:00 AM to 12:00 PM, providing easy access to vaccines during morning hours.
Registration and Vaccine Availability
To participate, individuals must register by calling Third Street Clinic at 419-522-6191. While vaccines will be available at each clinic, the type of COVID vaccines may vary. Those interested are encouraged to confirm availability during registration.
Assistance and Transportation
For individuals facing transportation barriers, the Area Agency on Aging offers help with travel arrangements. Call Susan at 567-247-6458 to schedule transportation. Additionally, attendees are asked to bring a form of ID and their insurance card to the clinic. Importantly, proof of citizenship or residency status is not required to receive a vaccine.

The Area Agency on Aging aims to break down the barriers that many Ohioans face when seeking vaccines, including issues related to transportation, location, or mobility. Through partnerships with local health providers and community organizations, this initiative ensures that all Ohio residents have access to essential vaccines.
