From Avita Health System
Reduced Cost Blood Screenings will be offered throughout the month of October.
To participate, a voucher must be purchased in the Cashier’s Office at any Avita Hospital or the Bellville Walk-In Clinic from October 3 through October 31.
The following vouchers will be offered:
- HealthLink Lab Profile – $30
(Lipids, Chems, CBC) - PSA – $25
(Prostate-Specific Antigen) - HgbA1c – $30
(Glycosylated Hemoglobin) - TSH – $15
(Thyroid Stimulating Hormone)
Vouchers can be redeemed during the month of October 2022 at any Avita Hospital or the Bellville Walk-In Clinic. Vouchers can be purchased and redeemed on the same day if testing preparation requirements are met.
Please note the following open hours:
Cashier Office Hours – Monday – Friday, 8:00 AM to 4:30 PM
Hospital Lab Hours – Monday – Friday, 6:00 AM to 5:00 PM; Saturday, 7:00 AM to Noon; Sunday, Closed
Bellville Walk-in Clinic Hours – Daily, 10:00 AM to 7:00 PM
PLEASE NOTE: Your physician/provider will NOT receive a copy of your lab report. Your results will be mailed directly to you. It is YOUR responsibility to share these results with your provider. Vouchers are not available for purchase or redemption at the Ontario Walk-In Clinic.
